Print and fill out the application form.
Paper copies are available at St. Anthony Preschool. To schedule an enrollment appointment, submit an interest form or call (414) 212-8199.
Return completed applications to St. Anthony Preschool at 1644 S. 9th Street, Milwaukee, WI 53204. Additional paperwork may be required in-person to complete your enrollment.

Tuition & Fees
Families enrolled in the Wisconsin Shares W2 program are required to pay a weekly co-pay for each child. Invoices will be issued every other Tuesday, and payment must be made in full at the preschool front desk by the specified due date.

Annual Registration Fee
A $100 non-refundable registration fee per child is required upon enrollment and annually thereafter. Financial assistance for this fee may be available through the Wisconsin Shares program.
Late Pick-Up Fee
St. Anthony Preschool and Daycare closes at 6:00 p.m. per state law.
A $1 per minute late fee applies for pickups after this time and must be paid by the next school day for continued attendance.
